Has your frenchie been administered Acepromazine and had a bad outcome? If so, please take a minute to complete the following form for each dog and/or incidence. I am trying to compile useful information for our breed. All information will be kept confidential and will only be used only to compile statistics. Please be as honest and specific as possible. Please do not include any doctors' or clinics' names. DOSAGES are extremely important. I will try to report my findings as information becomes available.

Did your frenchie have any breathing issues? specifically palate issues, stenotic nares, hypoplastic trachea?
Did your frenchie ever gag food, water or phlegm? Did your frenchie snore?
Did your frenchie have any spinal issues? Specifically muscle spasms, intervertebral disc disease, hemivertebrae, paralysis or paresis (weakness of limbs)?
Was your frenchie on any medication?
Please describe the episode that lead up to the administration of Acepromazine
Were other medications administered along with, before or after administration of Acepromazine?
What dosage was your frenchie given and what route (Intraveneously, orally, intramuscularly, subcutaneously)?
How long after administration of Acepromazine was an effect seen?
Did your frenchie have a positive or negative effect from the acepromazine?
Please describe the reaction and action taken
If the reaction was negative, did you file an Adverse reaction report with the FDA?
Date and Time of Death (if applicable)
